AWARD PRESENTATION
|
| |
Certificate of
award and a shield will be presented in presence of
several prominent personalities from government, diplomats
and industrial sectors in last week of Feb-2012. |

|
| SELECTION PROCESS |
| |
Nomination should be received along
with the applicable documents will be placed before a
panel of judges, nominated by CSRPK from the various reputed
organizations.
Decision of the judges’ panel will be final for the nomination
of the award. |

|
| ELIGIBILITY |
| |
Large medium and small scale industrial / service organizations
/ units of the following categories are eligible to be
considered for the award: |
|
Public sector undertakings / enterprises |
|
Private sector organizations / projects, process &
product. |
|
Units of government departments. |
|
Trade / professional associations. |
|
Consultancy organizations. |
|
Cooperatives and individuals. |

Applications will be evaluated on the basis of award
year performance (year 2011) and track record of three
preceding years. |

|
| Documents Required by email only
(as attachment in MS Word format) |
|
Completed nomination form |
|
Organistaion or company logo in color in various sizes
(both on paper and CD) |
|
If selected for award, name of the company / unit to
be printed on certificate / shield or trophy, should be
given on a separate page. |
|
Introduction of company or department (no more than
75 word) will be announced at the time of announcement
for the award. |
|
A message from the Chairman, MD or Head of Department
of CSR with color snaps regarding the event (no more than
75 word) |
|
payment of processing fee by cheque or scanned receipt
of bank deposit |

| AWARD RULES & REGULATIONS |
|
CSR Association of Pakistan ensure secrecy of the information
of award applications, however, CSRPK will not be liable
for any loss arising out of disclosure of information
furnished in the application. |
|
The documents submitted along with the application will
not be returned. |
|
CSRPK reserves the right to change time, date and place
of award ceremony. |
|
Decision of CSR Association of Pakistan regarding award
presentation shall be final. Any appeal / canvassing or
correspondence shall not be entertained. |

| SUBMISSION OF APPLICATION &
PROCESSING FEE |

The documents mentioned as may be applicable to your
type of industry; organization & project are to be
submitted in accessible email format: We may ask for a
copy of all documentation as soft copy CD where necessary.

Processing Fee PKR. 30,000/- (non-refundable) made payable
to our partner charity |
